Service Description

Wellways Well Connected provides non clinical support to individuals who are experiencing severe mental health. Support provided is individualised to the person’s personal needs and identified goals. This support is recovery focused and can include support to appointments, daily living skills, connection to the community, appropriate referrals, and collaboration with other service providers to get the best outcome to meet individual needs.

Well Connected provides outreach support to all areas of the Murrumbidgee region.

Eligibility

For people aged 16+ who experience severe mental illness and are not being supported by the National Disability Insurance Scheme (NDIS) funding.

Referral Process

Alliance referral form is used and sent to psychosocial@wellways.org
