Service Description

Well Connected provides on mental health recovery support to eligible people in the community, we focus on Aboriginal and Torres Strait Islander people and people over the age of 65 years.

Well Connected provides non clinical support to individuals who are experiencing severe mental health. The support Well Connected provides is individualised to the person’s personal needs and identified goals. This support is recovery focused and can include support, support to appointments, daily living skills, connection to the community, appropriate referrals, and collaboration with other service providers to get the best outcome to meet the individual needs.

Well Connected provides outreach support to all areas of the Murrumbidgee region.

Eligibility

Individuals experiencing a server mental illness and would benefit from psychosocial supports. Aboriginal and Torres Strait Islander people and people over the age of 65 years of age are our target cohort.

Referral Process

Alliance referral form is used and sent to psychosocial@wellways.org
