Service Description

Non clinical, individually tailored supports to an individual following a hospital presentation in the context of recent suicide attempt or suicidal crisis (plan & intent to die) for up to three months (12 weeks).

A Support Coordinator contacts the person within one business day of receiving their referral and tailors a program for up to three months after discharge from hospital. This program is built on personal connection and integration with community/clinical services.

Eligibility

A person must present to a hospital, emergency department or local community mental health service following either a suicide attempt or if they are currently deemed to be in suicidal crisis (ongoing suicidal thoughts with a plan or intent to die)

Referral Process

The Way Back referral for is to be completed and all referrals emailed to;

referrals.thewayback@wellways.org so that eligibility can be determined.
