Service description

MyStep to Mental Wellbeing (MyStep) offers a range of mental health supports, delivered face to face and via telehealth, and in-reach into residential aged care facilities.

MyStep services are based on a stepped care model, which is an evidence based, staged system that matches the level of intervention to the individual’s needs at the time. Services range from low intensity (brief interventions) to higher intensity (Team Care).

The in-reach services into residential aged care facilities offered under MyStep is for residents of aged care facilities who have, or are at risk of developing, a mental illness, such as depression, adjustment disorders or abnormal symptoms of grief and loss.

Eligibility

People living in the Eastern Murrumbidgee or accessing care via a GP in the region.

How do I organise an appointment or referral?

Anyone can refer to MyStep via MPHN Central Access and Navigation Service, by phoning 1800 931 603. Referrals can be faxed to:02 6921 9911 or sent by email:can@mphn.org.au

Alternatively people can contact GPH directly on 6453 4440 or complete a provisional referral form.
