Service description

  • Mental Health Community Nursing Program (MHCNP): The community based nursing program is designed to support people with severe and complex mental health care needs. Nurses work collaboratively with carers, families and other agencies to deliver recovery based mental health.
  • National Disability Insurance Scheme: A registered NDIS provider delivering support coordination, therapeutic supports, and a range of life skills development and community participation supports for people experiencing mental ill health.

Eligibility

  • MHCNP: To be eligible for this program individuals must:  have a severe mental illness that causes disruption to personal, social and/or work life; be supported by a general practitioner (GP) or psychiatrist but do not need ongoing support from the Community Mental Health Team (occasional contact and support is adequate); and have been hospitalised in the past, or are at risk of being hospitalised in the future.
  • NDIS: A person holding an NDIS plan, or a person with disability (physical and/or psychosocial) who has been assessed against the eligibility requirements of the NDIS as a suitable participant. Access requests can be found at your local NDIA office, or contact us for access support.
